Committee Reporting Forms

There should be one report per chapter submitted for the activities/events listed below. Once you have completed the form and click submit, a copy will be sent to the you via the email address you enter on the form, as well as the committee chair. Your copy should be included in your Achievement Week report as well.
NOTE: IT IS NOT THE RESPONISBILITY OF ANY COMMITTEE CHAIR TO FORWARD YOUR REPORT TO ANOTHER COMMITTEE CHAIR.

For the programs listed below there will be in most cases multiple events so there will be multiple reports that need to be completed. For these reports it is the responsibility of the chapter to:
  1. Complete a form for each event
  2. Retain a copy of the form (A pdf copy will be mailed to the email address you provide)
  3. Combine all the reports.
    • Example: If you had 10 Social Action Events, you will need to have completed a form for each event having a total of 10 social action reports
  4. The combined report should be emailed to the committee chair by August 1, of the reporting year.
  5. The combined report should also be included in the Achievement Week Report
